Is it "poor form" to not use a driver off the tee?

For a low handicap or scratch golfer, is it considered bad form to use a 3W or 5W off the tee instead of a driver? Also, is it bad form to not use an actual tee, and just hit the ball off the gr out of the tee box? Just wondering how these practices are viewed by higher-level golfers.
